How to Find Us

By car 

From points South and West 

Take Interstate 680 North and exit at Monument Boulevard (Exit 49). Use the right lane to merge onto Monument Boulevard. In 400 feet, turn right at the traffic light at Buskirk Avenue. Follow Buskirk Avenue past the Crossroads Shopping Center until you reach the signal at Buskirk Ave. and Clarie Dr. Proceed straight through the signal and onto Hookston Rd., then enter the Hookston Square complex just as the street begins to bend to the left. Once in the complex, stay to the left and park outside the 3480 building. Enter Suite 110 from the first walkway ramp on the exterior left side of the 3480 building. 

From points North and East

By BART 

Exit the Pleasant Hill station stop. Our new location is less than a mile way. Taxi and other car services are available at the BART station. To walk to our new location, exit the station on Wayne Drive and follow it a short distance until it meets Oak Road. Walk right on Oak Road and continue for about a half mile until it turns into Buskirk Avenue. In a few hundred feet, look for signs for Hookston Square at 3480 Buskirk Avenue and walk right into the complex. Follow signs for John Muir Health and the 3480 building address. Enter Suite 110 from the first walkway ramp on the exterior left side of the 3480 building. 

By bus 

Our new location is services regularly by the #18 and #316 County Connection bus lines, with a convenient stop directly outside our complex. Check the County Connection bus schedule for services days and times.

Lung Health Services Rehabilitation Program

This intensive seven-week education and exercise program is designed for people with chronic lung disease. A physician referral and fee may be required. Graduates can participate in an ongoing exercise program and monthly education and support sessions.

Respiratory Care

We provide pulmonary exercise testing, pulmonary function studies, and many other services. A fee is required.

Lung Disease Education/Support Group

A free, welcoming group for patients and their families meets at 2:00 PM on the last Wednesday of each month at the Concord Medical Center.

Sleep Apnea Patient Support Group

This free group supports sleep apnea sufferers, their loved ones, and other interested parties. It meets at the Walnut Creek Medical Center.

Pre and Post Lung Transplant Support Group

If you or your loved one are part of our unique community of Pre and Post Lung Transplant, you are invited to our Support Group! Come hear our stories, share your experiences, and get support when you need it from people who know what you are going through.

The Idiopathic Pulmonary Fibrosis Lung Disease Support Group provides an understanding environment where you will receive emotional support when you need it from people who know what you're going through. You will obtain strategies to help cope with your condition and have your questions answered.
